GENERAL PURPOSE OF THE JOB
This position is responsible for the delivery of all nursing services for Chronic Kidney Disease patients under their care.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive:
• Performs direct patient care, determined by the area of practice including but not limited to taking:
o Monitoring vital signs, physical assessment of patient
o Transcribing physician orders
o Drawing and documenting ordered laboratory work
o Documenting treatment and observations
o Administering medications according to physician orders
o Providing dialysis therapy (HD/PD)
o Providing vascular access (VA) care and assessment for HD
o Assisting with VA center procedures
o Providing appropriate information to the patient regarding their care
• Analyzes patient assessments, laboratory data and, when concerns are noted, brings issues to the attention of physician
• Communicates all emergencies, patient life-threatening complications and related incidents to the Head Nurse, Nephrologist, Director of Clinical Services and/or Regional Operations Director
• Assesses patient and family learning readiness and provides appropriate information
• Maintains a high standard of clinical practice and safe working environment
• Promotes and maintains safe infection prevention and control practices
• Instructs patients regarding dialysis, access care and chronic kidney disease (CKD)
• Instructs patients on pre-transplant preparation
• Administers patient quality care in a considerate, respectful manner
• Participates in long- and short-term care planning and monthly progress notes
• Attends mandatory training as part of the onboarding process
• Assumes responsibility for administrative/clerical duties as needed
